1. Direct Access & Scope of Care

In the State of Michigan, patients have direct access to physical therapy without a physician’s referral for up to 21 days or 10 visits, whichever occurs first. Continued treatment beyond this threshold will require a prescription/referral from an authorized physician, PA, or NP as defined under Michigan Public Health Code.

2. Cash-Based Practice & Payment Model

We are a fee-for-service / cash-based healthcare practice. Payment in full is due at the time services are rendered or in accordance with your chosen monthly subscription plan. We do not bill insurance companies directly. While we provide itemized Superbills with diagnostic and treatment codes upon request, we do not guarantee third-party reimbursement.

3. Cancellation & Attendance Policy

We provide exclusive, one-on-one time with a licensed Doctor of Physical Therapy. If you need to cancel or reschedule, please provide at least 24 hours notice so another patient can be accommodated. Late cancellations or unattended appointments may incur a cancellation fee.

4. Medical Advice Disclaimer

2. No Surprises Act & Good Faith Estimate (GFE)

Under the federal No Surprises Act (effective January 1, 2022), healthcare providers must give patients who are uninsured or self-pay a Good Faith Estimate of the expected charges for non-emergency medical services. Because our practice follows a direct, transparent fee structure, you will always receive full, upfront cost details before any assessment or treatment starts. There are no surprise facility fees or unexpected post-treatment charges.

3. State of Michigan Direct Access Compliance

Under Michigan Public Act 264 (MCL 333.17820):

• Patients may be evaluated and treated by a licensed Doctor of Physical Therapy without a prescription for up to 21 days or 10 treatment visits.
• If treatment continues beyond 21 days or 10 visits, we will coordinate directly with your primary care physician or appropriate healthcare provider to secure the necessary prescription.

4. Clinical Licensing & Evidence-Based Practice

All clinical care is administered by licensed Doctors of Physical Therapy registered with the Michigan Board of Physical Therapy (LARA). Treatments follow peer-reviewed orthopedic protocols and ethical physical therapy practice standards.
